CHAP (Community Health Accreditation Program) is an independent, nonprofit accrediting body for home and community-based health care organizations. CHAP accreditation is a sign of the continuing advancement of quality on which millions of consumers rely. Our trained care coordinators identify specific needs of each client and design a program to enhance the quality of life for the client and family. We perform initial assessments and make periodic visits to monitor progress and client satisfaction.
